Lately I have been feeling very down. I currently live in an assisted living private home. I have self harmed before and currently have thoughts of doing it again. I take Zoloft, but I feel I need something stronger. I have a few health issues I am dealing with. Currently I suffer from Diabetes, high blood pressure, urine incontinence, poor blood circulation which sometimes causes swelling, high cholesterol, and on nine medications. Many days when I feel I can barely get out of bed, I just want to scream. I was very active, did public speaking and worked as a bookkeeper. So sometimes I feel it is not worth getting out of bed.

Oh! That's great! (If you can tolerate waiting up to 10 days....) Do you want any suggestions on how to talk to your primary about this? (I'm an old hand with this). Ignore this if you're not interested: Write it all down in advance. Write all symptoms - anxiety, depression, history of self-harm, desire to do it again, want to scream with despair, can barely get out of bed with depression, loss of old active life, etc. Don't underplay what sort of things you're feeling - if in doubt, do leave it in, and your primary can decide if they want to include it in the writeup or not. I think you have a good case for therapy. Best of luck.
