NEWLY DIAGNOSED: Keep a medication list

Almost inevitably, cancer patients will be put on a range of different medications, and it's important that you keep a list of what you're taking, what doctor wrote it (if any), the dosage and the frequency. This goes for pain medication, chemotherapy drugs, medications to help with the side effects of chemo, over-the-counter drugs you're taking, and even herbal supplements. Such a medication list should be similar to the one below, which can be seen in a larger form and printed if you click on it.

Such a list should also be included in your treatment folder.

Thanks Ross this is an excellent form for everyone to have when they go to the MD office, you don't know how many times I have asked what are your current meds and the answer I get is "I can't remember". I printed this for my family to have for their appts. Knowing what you are on and what for is very important.

With our chemo brains running wild heck I need more than a med list Ross
I bought a mini laptop...6 inch jobber keeps my calendar and notes on and
gives me something to mess with in the dr office too :) but med list is right
at the top of first questions ask by anyone you see for sure. Awsome advice

people will forget what you said, they will forget what you did, but they will never forget how you made them feel...God Bless
